Protection of personal information
COTTON CLUB
Policies regarding the protection of personal information at the Club, based on relevant laws include:
1、Use of personal information for profit by Cotton Club Japan (Japanese Law, Articles 18 and 24)
The main kinds of data and their use by the Club are as stated below.
Principles regarding us of personal information obtained directly from customers themselves are explained here.

2、Third party use of personal information
The Club adheres to strict policies regarding the personal information at our disposal, and do not release such information without the client’s consent/approval. Exceptions to this policy include the following scenarios:
(1)(Overriding) legal considerations
(2)When the protection of human life, health, and personal assets are at stake and direct consent
from relevant person(s) cannot easily be obtained.
(3)When public heath or the health and welfare of children is at stake,
and obtaining customer consent is difficult.
(4)When cooperation with national and/or regional public authorities,
or agencies entrusted by such authorities undertaking lawful action takes precedent, and obtaining customer consent represents an obstacle to such processes.
In addition, there may be extraordinary cases may arise when entrusting third parties with personal information is warranted. Under such a case, special consideration will be given to ensuring the safe handling and maintenance of the information in question.

3、Disclosure, correction, usage halt, etc. procedures (Law, Article 29)
(1)Information disclosure
In the event that the Club is asked to disclose personal information, we ask that the following protocol be observed:
a. Please apply through the information department set up for this purpose.
Proper forms for obtaining the sought after information, along with return envelopes will be mailed to you. Along with the details of your request, please mail to the Club documents that establish your identity or the identity of those persons being represented, along with the fee (a postage stamp worth approximately 800 yen).
The Club uses specially designed billing forms for processing requests. For information about business relationships, etc. forms may differ, so please inquire about which will be necessary in your case.
Disclosure request forms and return envelopes will be sent to you. Please fill the forms out as necessary, and provide relevant documentation that will verify your identity (see ※Caution 1 below for the party in question, and ※Caution 2 below for representative. The customer shall bear the cost of postage
(※Caution 1)Party in question identity verification
In the case that the party in question makes a direct request, materials that will help verify his or her identity will be necessary. For Japanese citizens, this may be a census registry form (domicile or another form that does not contain family information). In the case of a foreigner, proof of alien registration will suffice.

In the case that the party in question makes a request through a representative, materials that will help verify his or her identity will be necessary. For Japanese citizens, this may be a census registry form (domicile or another form that does not contain family information). In the case of a foreigner, proof of alien registration will suffice.
b. Notification from the Club/fees
Notification from the Club will be sent by post to the address listed on the applicant’s census registry form.
c. Replies regarding information about show reservations will be provided only to the party
in question and only when his/her identification has been proper verified.
d. Disclosure processing refusal
Please be aware in advance that in the event of reasonable harm or damage to the proprietary operations of the Club where proper identity verification is not possible, information disclosure will not be processed.
(2)Request for personal information correction/halt of use
In the event that the Club is asked to correct or stop using personal information, we ask that the following protocol be observed:
a. Direct request by the affected party
Direct inquiries are handled by the Club by either telephone or post. Please provide information/materials that will help the Club determine the authenticity of your identity at the time it was registered (e.g. name, address, telephone number).
b. Request by representative
Requests made by representative are handled by post only. We will send the forms to you
・Address, name, telephone number and correction form
・Reason for information use stoppage (freestyle form)
・The following proof of ID/representative power documentation,
in addition to census registry OR foreigner registration (whichever applies) will be required in the case that either the inquirer himself/herself or his/her representative applies for disclosure of information:
・In the case the representative is determined by charge of trust,
a properly stamped proof of trust document is required.
・In cases involving representation by legal counsel,
documents certifying such representation, such as proof of census registry.
・Materials used to confirm the identity of client representatives (proof of citizen registry, etc.)
will be disposed of in a speedy and appropriate manner.
※Please note that materials submitted to the Club will not be returned.
c. About the request process
Please contact the same desk as indicated at the time you registered your personal information.
d. Club Notification methods & fees
The Club will make contact with the affected indivisuals or their representatives by telephone, post, etc. This is done free of charge.
e. When correction/usage halt cannot be processed
Order processing may not be honored in the event that the identity of the persons affected or their representatives cannot be confirmed, or if discrepancies in the information change request and pertinent files differ. In such a case, the Club will notify the parties which made the request.

4、Policy revisions
The policies above are subject to revision. Such changes will be publicly announced.
